About the Role
We are seeking dedicated and compassionate Case Managers to join our team in the Bronx and Brooklyn. In this role, you will support clients by connecting them with essential services, coordinating resources, and helping them achieve stability and long-term success. This is an excellent opportunity for someone passionate about community work and looking to grow in the human services field.
Key Responsibilities

  • Provide case management services to a diverse caseload of clients

  • Conduct assessments, develop service plans, and track progress

  • Connect clients to appropriate programs, resources, and community supports

  • Maintain detailed documentation and case notes in accordance with agency and regulatory standards

  • Collaborate with internal teams and external partners to ensure coordinated care

  • Participate in team meetings, trainings, and outreach activities as needed
Qualifications

  • Bachelor’s degree or at least 2 years of relevant case management or social services experience

  • Strong communication, organizational, and documentation skills

  • Ability to work with diverse populations in a fast-paced environment

  • Familiarity with social service systems in NYC is a plus
