RN Case Manager –Hospice (Home Health)
Location: Toledo, Ohio
Pay: $40–$42 per hour
Schedule: Full-time | Flexible shifts with on-call rotation

We’re seeking a dedicated Registered Nurse (RN) Case Manager –Hospice to plan, coordinate, and deliver high-quality hospice care in patients’homes and facilities. This role ensures compassionate end-of-life care while maintaining compliance with all state, federal, and company standards.

Key Responsibilities:

  • Provide ongoing nursing assessment, symptom management, and care coordination for hospice patients

  • Develop and implement individualized Plans of Care and evaluate their effectiveness

  • Communicate proactively with physicians, caregivers, and the interdisciplinary team regarding patient needs

  • Educate patients and families to ensure understanding and comfort

  • Maintain accurate and compliant documentation in EMR systems

  • Participate in quality improvement and compliance initiatives

Qualifications:

  • Current, unencumbered Ohio RN license (required)

  • Minimum 1 year of acute care or med-surg experience (hospice preferred)

  • Proficiency in Microsoft Office and EMR systems

  • Valid driver’s license, reliable transportation, and insurable driving record

  • Strong communication and organizational skills
