Now Hiring: RN Telephonic Clinical Care Managers –Longitudinal & Palliative | Fully Remote | $62/hour

We are seeking experienced Registered Nurses (RNs) with a NY license to join our team as Longitudinal and Palliative Case Managers. Provide high-quality, telephonic clinical care management to help patients remain safely in their homes or communities while delivering evidence-based support.
Key Responsibilities

  • Coordinate care and services for assigned members

  • Complete comprehensive care management and disease-specific assessments

  • Conduct timely telephonic calls based on patient risk levels

  • Provide health education, coaching, and support for adherence to treatment plans

  • Address social determinants of health and connect members to resources

  • Identify safety issues and connect to therapy, DME, behavioral health, etc.

  • Participate in interdisciplinary team (IDT) meetings and case discussions

  • Maintain confidentiality and ensure compliance with regulations
Qualifications

  • Registered Nurse (RN) with active NY license —Required

  • Experience in longitudinal or palliative care case management

  • Knowledge of Medicare and Medicaid programs

  • Strong communication, problem-solving, and patient advocacy skills

Pay: $62/hour | Fully Remote
